Abstract This study focuses on to identify the various procedures at the outpatient clinic as well as to investigate the possible operational problems that may lead to excessive patients’ waiting time. A patients’ experience in waiting time will radically influence his / her perceptions on quality of the service. The study was carried out in Fortis Memorial Research Institute, Gurgaon (Haryana) for three months. The subjects were outpatients who came to the outpatient clinic in FMRI. Data was analysed using the six sigma approach.Significant reduction in waiting time was achieved in the outpatient services of department by using the six sigma approach. Doctors were on time in OPD to reduce the delay in the OPD. Patients were coordinated well to the doctors. SIX SIGMA First introduced by Motorola in 1986, Six sigma is a method to measure the quality of a process to fulfil customers’ needs which approaches perfection. Data and statistical analysis were used to identify defects in processes and reduce variation. With Six sigma, defects in a process can be measured by identifying the best method to eliminate defects and approach ‘zero defect’. Customers’ satisfaction is the main factor. Fig. 10: Waiting Time in OPD 93% patients felt that they were well informed regarding waiting time in OPD whereas 7% patients felt that they weren’t informed well. Fig. 11: Information regarding Waiting Time As per as interaction with doctor after arrival is concerned, out of 150 patients 98 patients interacted within 10 minutes, 12 patients interacted within 10-20 minutes, 26 patients interacted within 20-30 minutes and 14 patients interacted more than 30 minutes. In this step, accurate metrics have to be used to define a baseline for further improvements. The main reasons behind it are: Delay in Doctors’ in time( e.g. Doctor in OT/ On Rounds) Delay in Investigations. Long consultation time. Month October- It was observed, Average Time = 15 minutes. No. of patients seen >15 minutes = approx. 16 % of no. of patients seen > 15 minutes = 30.19%. Fig. 18: Observation in Month October In this month slight improvement was observed as the average time, no. of patients seen more than 15 minutes and its %age decreased slightly. This was due to: Doctors requested to be on time so that patients can’t feel inconvenient. Complete consultation on time. COMPARISON AMONG MONTHS AUGUST, SEPTEMBER OCTOBER: Table.5: Comparison among Months August, September October It was observed when compared the data of three months that: Average waiting time was decreased followed by slight increase. Maximum time for the consultation has decreased within three months. Minimum time was approximately zero in these three months. Fig. 19: Comparison among months August, September October IMPROVE Improvements in existing systems are necessary to bring the organization towards achievement of the organization goals. Creative development of processes and tools brings about a new lease on life for the organizations processes and takes them nearer to organizational objectives. Various project management and planning tools can be used to implement these new techniques and processes. Appropriate usage of statistical tools is important to measure the data, which is necessary to understand improvements done and any shortcomings that may exist. The solutions with their respective Causes are shown below: Table.6: Causes Solutions CONTROL Control phase is the last step in the DMAIC method. This phase is about sustaining the changes made in the Improve phase to guarantee lasting results. The best controls are MONITORING, appropriate CHECKS and balance the quality system for the long run. Recommendations Segregated OPD counters for billing. Separated nursing station for the departments to reduce waiting time. OPD schedule to be blocked for the time duration when the doctors are in procedure or on rounds. Time slot for appointments to be increased from 10 to 20 minutes per doctor. Conclusion: Significant reduction in waiting time was achieved in the outpatient services of the department. Doctors were on time in OPD to reduce the delay in the OPD. Patients were coordinated well to the doctors. Further data analysis will help to monitor and control the waiting time and process flow in the OPD.

Depending on what your beliefs are the reason for the ever so shortening or longing of years could have something to do with spirituality as well as environmental conditions. The demographic transition theory shows how the population is changed throughout time.It is based on the study and documentation of changes in birth/death rates. These significant transitions Result from low-high birth/death rates to high-low birth/death rates. When the aging population begins to rapidly increase, it usually is because of less new-birth but also less death. Low birth rates are as a result of access to contraception, preference to smaller families, urbanization and education of women. What a person prefers is often their business and left to their discretion, but with less youth, there are less younger adults that are able to take care of the older generation that may be needing help. However, we should remember that we have to keep an attitude and a balance of the population and the resources available to supply the population.. There are upsides to the increase in the aging population but there are also downsides. The pros could include the fact that elders need to be attended to more could open the door for young adults to find jobs, also being guided by successful elders who hold secrets to prospering is something no one can deny is very helpful, also elders hold to a sense of familiarity meaning that just when all the fads and trends calm down, elders are there to stabilize the air. As for the downsides of the aging population, the economy is not as able to grow properly because the work field is going down, also health care has increased costs as it relates to treatments and medical care that is always needed for old people, and to add with more machinated tasks being carried out by machines as we ll as younger workers those skilled to work in a trade or a craft are not needed. Elders are having to make a choice between personal care from loved ones or the impersonal care that comes from nursing homes, home health nurses, or extended care facilities. Sadly, because of the lack of wanting to burden family members and the like, elders usually choose care from the list of impersonal care and internalize the neglect felt from having to do so. Advantages and disadvantages of implications of an aging population must be examined so that plans of implementation can be properly executed and adopted to fix or balance the situation.  The impact the older population is making does not have to be the be-all end-all factor of what is to happen as predicted by many. Increased dependency is believed to be one of the heaviest burdens that will cause the elderly to cripple the economy but it does not have to be so. We will also use the aforementioned to discuss the actions that must be taken to address the issue of the aging population. As well as having to rely on the government to make sure that this problem that seems to have arisen is taken care of there are things that we can do to be effective as well. We can definitely make sure our elders don’t feel like burdens. When I was younger we were able to adopt a grandparent. They could have kids or grandkids but many did not have any that would visit often or at all. We  would be allowed to spend time with them every weekend and it would just brighten their day to hear about what we are allowed to do and what we would discuss with them. Something as simple as this wakes the elders mind up in the nursing homes, it gets them excited again about life and their mind off where they are. Of course more financial and secure measures need to be taken but with just a simple economical start we could very well being on the road to the discovery of what works. Narcissistic Personality Disorder is a complicated and often misunderstood mental disorder. Narcissistic personality disorder is a psychological in which an individual shows and abnormal obsession with themselves. Narcissism can be easily confused with simple arrogance, but the disorder can range from obsession to a life-consuming neurotic compulsion. The individual suffering from Narcissistic Personality Disorder uses their extreme ego to cover their sense of inferiority, which is the real problem of narcissist; To make the matters worse, for patients suffering from NPD, the disorder is under heavy scrutiny from some psychologist who do not believe it is a sufficient diagnosis and was almost taken out of the DSM. I believe that Narcissistic Personality Disorder is a disorder that should be treated to the same extent as other Mental disorders in the DSM. According to the Mayo Clinic, Narcissistic personality disorder is somewhat new. The term was invented in 1967 to describe an over expression of the narcissism trait that has been recognized since ancient times. the extreme confidence that is exhibited by a person with narcissistic PD is just a front. People with this disorder usually suffer from fragile self-esteem, and they attempt to cover for this by making themselves appear superior to those around them. The cause of Narcissistic Personality Disorder is unknown, although it generally begins to show itself in the early stages of adulthood. People afflicted with this disorder are generally known to be aggressive when confronted with criticism, and they lack the ability to accept their own failure. They are usually manipulative and find it difficult to maintain relationships because of their tendency to use people for their own ends. They exaggerate their talents, using self-glorifying methods to gain attention for themselves a t the expense of others. Intervention Strategies for Students at Risk Teens who are considered to be at-risk have a plethora of issues that need to be addressed, and learning in school is only one of them. By working  with these teens by using effective intervention strategies for studying and learning, its possible to help guide them on the right educational course. Directions or Instructions Make sure directions and/or instructions are given in limited numbers. Give directions/instructions verbally and in simple written format. Ask students to repeat the instructions or directions to ensure understanding occurs. Check back with the student to ensure he/she hasnt forgotten. It is a rare event for students at risk to be able to remember more than 3 things at once. Chunk your information, when 2 things are done, move to the next two. Peer Support Sometimes, all you have to do is assign a peer to help keep a student at risk on task. Peers can help build confidence in other students by assisting in peer  learning. Many teachers use the ask 3 before me approach. This is fine, however, a student at risk may have to have a specific student or two to ask. Set this up for the student so he/she knows who to ask for clarification before going to you. Assignments The student at risk will need many assignments modified or reduced. Always ask yourself, How can I modify this assignment to ensure the students at risk are able to complete it?  Sometimes youll simplify the task, reduce the length of the assignment or allow for a different mode of delivery. For instance, many students may hand something in, the at-risk student may make jot notes and give you the information verbally, or it just may be that you will need to assign an alternate assignment. Increase One to One Time Students at risk will require more of your time. When other students are working, always touch base with your students at risk and find out if theyre on track or needing some additional support. A few minutes here and there will go a long way to intervene as the need presents itself. Contracts It helps to have a working contract between you and your students at risk. This helps prioritize the tasks that need to be done and ensure completion happens. Each day, write down what needs to be completed, as the tasks are done, provide a checkmark or happy face. The goal of using contracts is to eventually have the student come to you for completion sign-offs. You may wish to have reward systems in place also.​ Hands-On As much as possible, think in concrete terms and provide hands-on tasks. This means a child doing math may require a calculator or counters. The child may need to tape record comprehension activities instead of writing them. A child may have to listen to a story being read instead of reading it him/herself. Always ask yourself if the child should have an alternate mode or additional learning materials to address the learning activity. Tests/Assessments Tests can be done orally if need be. Have an assistant help with testing situations. Break tests down in smaller increments by having a portion of the test in the morning, another portion after lunch and the final part the next day. Keep in mind, a student at risk often has a shortened attention span. Seating Where are your students at risk? Hopefully, they are near a helping peer or with quick access to the teacher. Those with hearing or sight issues need to be close to the instruction which often means near the front. Parental Involvement Planned intervention means involving parents. Do you have an agenda in place that goes home each night? Are parents also signing the agenda or contracts you have set up? How are you involving parental support at home for homework or additional follow up? A Strategy Summary Planned interventions are far superior to remediation approaches. Always plan to address students at risk in your learning tasks, instructions, and directions. Try to anticipate where the needs will be and then address them. Intervene as much as possible to support students at risk. If your intervention strategies are working, continue to use them. If theyre not working, plan for new interventions that will help students succeed. Always have a plan in place for those students who are at risk. What will you do for the students that arent learning? Students at risk are really students of promise be their hero.
